Your Voice Shapes the Year Ahead

Every member of the Rotary Club of Kingsport is invited to complete this year’s Annual Membership Feedback Survey.

Whether you attend weekly, participate when you can, or are stepping back in after time away, your reflections belong in this conversation.

The survey takes approximately three minutes to complete. It includes six short questions, and none of them are required. Answer what feels meaningful to you.

Visit kingsportrotaryclub.org/survey, enter the email address we have on file for you, and complete the survey at your own pace.

Your response is anonymous by default. You may include your name if you would like a club leader to follow up with you.

On Paper

Prefer a pen and a quiet chair? Download the printable survey PDF and return it using one of the following options:
